Verification of Benefits: $50.00 fee per verification of benefits.
Flat Rate Fee: $350.00-This fee is for the creation of itemized receipts. This fee includes my time assisting the member in completing the claims filing process. Appeals, audits and phone calls with the claims department are included in my flat rate consultation fee.
Group Package Flat Rate Fee: $550.00-This fee is designed specifically for individuals who utilize multiple out of network maternity care providers. Examples include, chiropractors, pelvic floor therapy, lactation consultants, midwives, birth center facility fees, doula care and OB/GYNs.
Midwife Contracts: 10% of insurance payments. This fee only applies to the midwifery groups I contract with who accept insurance.
